Youth Who Are 16 Years And Above

2. Certified copy of one of the parent/legal guardian/informant’s identity document
3. Death certificate or certified copy if parents are deceased
4. Proof of residence (where it is available) or provide a residential address
5. Free of charge

Pensioners Who Are 60 Years And Above

1. Green bar-coded identity document/affidavit in case the identity document is lost
2. Proof of residence (where it is available) or provide a residential address
3. Marriage certificate, if married (for marriage verification status)
4. Divorce decree, if divorced (for divorce verification status)
5. Free of charge

Other Citizens
